Chapter 2 . 4 Somatosensory evoked potentials

چکیده
Somatosensory evoked potentials (SEPs) are the electrical potentials generated in sensory pathways at peripheral, spinal, subcortical and cortical levels of the nervous system. SEPs can be elicited from almost any nerve, although the median and posterior tibial nerves are usually chosen in clinical practice. Recently up-dated reviews on SEP recording, normal waveforms, normative data and clinical applications can be found in Jones (1993) and MauguieÁre (1999). Recommendations from an IFCN committee published in 1994 (Nuwer et al. 1994) have been updated in this chapter. The short latency responses occur within the ®rst 60 ms after an electrical stimulus. Later middle latency and long latency SEPs show a wider range of normal variability making their clinical use more dif®cult. SEPs are usually evoked by bipolar transcutaneous electrical stimulation applied on the skin over the trajectory of peripheral nerves. Electrical stimuli used in clinical practice (see below) produce a twitch in the muscles innervated by the stimulated nerve when it contains a contingent of motor ®bers. At this intensity the rapidly conducting large myelinated ®bers, including ®bers subserving touch and joint sensation but also muscle afferents, are activated because of their higher resistance. Muscle afferents have been shown to elicit cortical SEPs 79 Recommendations for the Practice of Clinical Neurophysiology: Guidelines of the International Federation of Clinical Physiology (EEG Suppl. 52) Editors: G. Predictive value of somatosensory evoked potentials for awakening from coma.
OBJECTIVES A systematic review of somatosensory evoked potentials performed early after onset of coma, to predict the likelihood of nonawakening. The pooled results were evaluated for rates of awakening, confidence intervals, and the possibility of rare exceptions. DATA SOURCES Forty-one articles reporting somatosensory evoked potentials in comatose patients and subsequent outcomes, from 1983...
متن کاملVisual and spinal evoked potentials in diagnosis of multiple sclerosis.
The measurement of cortical evoked potentials generated by pattern reversal is a sensitive technique for detecting demyelinating lesions in the anterior visual pathways.1 2 Similarly changes in the somatosensory evoked potential recorded over the cervical spine3 have been found in many patients with multiple sclerosis (MS).4 We have assessed the relative and complementary values of these two te...
متن کاملEffects of 2% lignocaine on somatosensory evoked potentials recorded in the extradural space.
We have studied the effects of extradural administration of 2% lignocaine at the L3-4 interspace on somatosensory evoked potentials recorded in the cervical extradural space before corrective surgery for idiopathic adolescent scoliosis. Eight patients in whom the equivalent volume of 0.9% sodium chloride solution was administered into the lumbar extradural space acted as a control group. Lignoc...
متن کاملفراوانی پتانسیلهای برانگیخته غیر طبیعی در مالتیپل اسکلروزیس: مطالعه مقطعی در 25 بیمار
Background: Visual, brain stem auditory and somatosensory evoked potentials (EPs) have been traditional paraclinical tests to evaluate the competency of sensory tracts in multiple sclerosis (MS) patients. It seems that only one of these EPs could be sufficient, at least as a screening test. The objective of this paper is to evaluate the frequency of these three evoked potentials in definite MS ...
متن کاملExploring the Mechanisms of Exercise-Induced Hypoalgesia Using Somatosensory and Laser Evoked Potentials
Exercise-induced hypoalgesia is well described, but the underlying mechanisms are unclear. The aim of this study was to examine the effect of exercise on somatosensory evoked potentials, laser evoked potentials, pressure pain thresholds and heat pain thresholds. These were recorded before and after 3-min of isometric elbow flexion exercise at 40% of the participant's maximal voluntary force, or...
